What is the physical exam consist of

The purpose of the Physician’s Statement is to determine whether the patient is physically, emotionally, and mentally able to provide care for a foster/adoptive child. Responsibilities may include 24-hour supervision, personal care, transportation, positive behavior management, providing follow-up care and medical treatment, and administering medication.( State of Arizona)

Components of the physical examination:

  • Evaluation of the patient’s general physical health
  • Evaluation of the patient’s general emotional health
  • a review of over the counter medications, and prescription medications and a determination if any of the over-the-counter or prescription medications regularly used by the patient would interfere with the safe care and supervision of children
  • Review the Health Self Disclosure form (CSO-1232A) 
  • Does the patient present with or is known to have communicable diseases
These evaluations/physicals generally take between 30 and 45 minutes. A physical exam will be performed and  a discussion of the patient’s medical history will be performed.
